Welcome to JWP’s Early Childhood Screening Program!
What is Early Childhood Screening?
Early Childhood Screening is a free health and development check of how your child is growing and developing. Early Childhood Screening includes a vision and hearing check, height and weight, a review of health history and immunizations, and an age-appropriate developmental screening. While Early Childhood Screening includes a vision screening that helps detect potential eye problems, it is not a substitute for a comprehensive eye exam.
**A developmental screening and up-to-date immunization records are required by Minnesota law PRIOR to kindergarten entrance.
What happens at Early Childhood Screening?
Once you have signed your child up for Early Childhood Screening there will be important forms for you to complete prior to your child being screened. These forms are available right here on our website under Early Childhood Screening Forms, or can be sent to you in the mail! In order to ensure a more efficient screening process, please have all of these forms complete BEFORE your child’s screening appointment.
On the day of your child’s screening, please arrive 5-10 minutes early to get checked in and complete the registration process. Early Childhood Screening is located at the JWP school, 110 E. 3rd St. Janesville, MN 56048. You and your child will check in at the district office located through door S1. Once checked in an Early Childhood school district staff will call your child’s name, introduce themselves, collect your completed forms, and invite you to come and begin the screening process.
The screener has a series of questions that will be specifically asked to either you or your child and have different tasks that your child will complete. You are welcome to watch your child but are asked not to participate in any way. Lastly, a teacher will complete an exit interview with you. This is a review of the screening results and the forms you completed. This is also your opportunity to bring up any questions or concerns you may have about your child. You will receive a copy of your child’s screening summary and the screening process is complete!
**PLEASE NOTE: If an adult other than the parent or legal guardian will be bringing your child in for screening a release of information MUST be filled out, signed by you, and given to Early Childhood school district staff PRIOR to the start of screening or your child’s screening appointment will be canceled. You will also need to state whether you would like the screening results shared with this person otherwise the exit interview process will be skipped and completed with you over the phone.
If you have any questions about the screening process feel free to ask the Early Childhood Screening staff during your child’s screenings or contact Lisa Schroeder, Early Childhood Screening Coordinator at 507-231-2102 or email@example.com.
